Comprehensive DBT Explained: Skills, Support, and Real-Life Change

If you’ve heard of Dialectical Behavior Therapy (DBT), you might think of it as just another form of talk therapy. But Comprehensive DBT is much more than weekly sessions—it’s a structured, evidence-based treatment model designed to help people make real, lasting changes in their lives. Comprehensive DBT is made up of four key components: individual therapy, skills training, phone coaching, and a DBT consultation team. Each element serves a specific purpose and works in tandem to reinforce progress.

DBT individual sessions are structured, purposeful, and focused on helping you understand your behaviors, navigate emotions, and make meaningful changes in your life. These aren’t open-ended “just talk” therapy sessions. Your therapist actively guides the process, offers validation and support, and helps you move toward the life you want.

Each session starts by reviewing your diary card—your tool for tracking emotions, urges, behaviors, and skills throughout the week. From there, you and your therapist set an agenda for the session, guided by

DBT’s prioritization framework:

  • Life-threatening behaviors (such as suicidal thoughts or actions)
  • Therapy-interfering behaviors (like missing sessions, shutting down, or avoiding topics)
  • Quality-of-life interfering behaviors (such as impulsivity, relationship struggles, or emotional outbursts)

Together, you’ll look closely at patterns that keep you stuck—what happened, why it happened, and what can be done differently next time. You might walk through a behavior chain, explore core beliefs, build motivation, or learn how to respond differently in the future.

While some skill application and teaching may occur, individual DBT therapy sessions are not just about skills—it’s about helping you understand and change the behaviors and emotional patterns that interfere with your life. Your therapist will help you stay focused on your goals, develop insight, and take steps toward building a life that feels more stable, connected, and worth living.

What Other Approaches Are Included In Comprehensive DBT?

In addition to individual sessions, Comprehensive DBT includes three essential components that work together to support lasting change: DBT Skills Training, Phone Coaching, and the DBT Consultation Team.

DBT Skills Training equips clients with practical tools to better manage emotions, navigate stress, strengthen relationships, and remain grounded in the present moment. Skills are taught in four core areas: Mindfulness, Distress Tolerance, Emotion Regulation, and Interpersonal Effectiveness. These sessions are available online for adults in Nevada, California, and Oregon. In-person groups are also offered in Las Vegas, NV. Individual skills sessions are also available for those who prefer a one-on-one format.

Phone Coaching provides real-time, brief support between sessions. It’s not therapy by phone, but rather focused, goal-oriented coaching to help clients apply DBT skills in everyday moments of distress or uncertainty. This bridge between sessions helps reinforce skill use in real-life situations. Especially during moments when clients feel overwhelmed or unsure of what to do next. Coaching gives you in-the-moment support to pause, check the facts, and choose skillful responses when things feel overwhelming or unclear.

The DBT Consultation Team

The DBT Consultation Team supports therapists in providing high-quality care and staying true to the model. This structured team gives therapists space to receive feedback, stay accountable, and continue growing their clinical skills. Just as clients benefit from structured support, therapists also thrive in a collaborative, growth-oriented environment. This kind of support helps maintain the integrity and effectiveness of DBT.

Together, these components form a cohesive, research-backed treatment model designed to help clients reduce harmful behaviors, build healthier coping strategies, and create more stable, meaningful lives. Treatment is always tailored to the individual, with clients and therapists deciding together which components best fit the client’s goals and stage of healing.

What Are the Downsides of DBT Therapy?

DBT is a powerful and effective therapy, and it’s also a commitment. Like any treatment, it’s not the right fit for everyone, and it comes with challenges that are important to understand from the start.

Here are some potential downsides of DBT therapy:

It requires time and structure.

Full DBT involves weekly individual therapy, weekly skills group, and completing diary cards between sessions. For some people, this level of structure can feel overwhelming at first, especially if they’re used to less intensive therapy.

It’s an active therapy.

DBT isn’t about venting or passively receiving insight. You’ll be asked to track your emotions and behaviors, look closely at patterns, and practice skills in real life. That can be uncomfortable at times, and also deeply rewarding.

It asks you to stay.

When emotions get intense or relationships feel hard, you may want to shut down or leave. DBT encourages you to stay in the process, even when it’s hard. That’s part of what makes it so effective—and it’s also part of what makes it challenging.

It might not feel validating at every moment.

DBT therapists work dialectically—they balance validation and change. That means you’ll be understood and supported, and you’ll also be asked to shift what isn’t working. That dual focus can feel frustrating at times, especially when what you want is comfort. Over time, though, many clients say this is what helped them grow.

At its core, DBT is about building a life worth living, not just reducing symptoms. That process takes work, time, and willingness. It’s not always easy, and for many people, it’s absolutely worth it.
